What will happen with my benefits?
All employee benefits will end at the end of the pay period in which your last worked day falls.
- Health and Dental benefits – Health and dental benefits may be extended through the COBRA regulations. If you are eligible for COBRA benefits, information will be sent via mail by Chard Snyder following the receipt of your final paycheck. All documentation must be returned to Chard Snyder within the provided time frame for COBRA benefits to be effective.
- Retirement Accounts - Retirement distribution packages will be sent following your final paycheck from Charles Schwab, for any individuals that have retirement accounts through Carle. For questions regarding your retirement account options, please reach out to Busey Wealth Management at 217-365-4874.
- Paid Leave Balances - Upon leaving Carle, your paid time off balance will be paid out with your last paycheck. It will be taxed at a flat 22% per IRS guidelines. Your sick bank and Personal Holiday balances are not paid out upon leaving Carle.
- Life Insurance - Life Insurance coverage will cease the final day of employment. Team members may be eligible to take their Life Insurance with them following termination of employment. Please complete the Life Insurance Conversion form and return to The Hartford within the required timeframe.
What if I have an outstanding referral bonus payment?
Both the referring and the referred employee must be actively employed at the time of the Referral payment. If either person terminates prior to the due date, the remaining payment is forfeited.
Do I need to repay my tuition assistance?
Payment may be required under certain conditions. Please contact Employee Relations at (217) 902- 5316 for specific information.
Did I receive a sign-on or relocation bonus that may need repayment?
If you received a sign on or relocation bonus and have not fulfilled the employment commitment, the remaining cash advancement amount not earned at the time of separation must be repaid. In addition, this amount due may be withheld from your final paycheck.
