You can sign up for Health and Dental Insurance during Carle's open enrollment period, which occurs each fall for a January 1 effective date.

You may also enroll, disenroll, or make changes to your Health and Dental plan within 31 days of a qualifying life-change event, which includes marriage, divorce, birth or adoption of a child, death of my spouse or child, employment or termination of employment of my spouse, or a status change from part-time to full-time or vice versa.

• Use the Benefit Change Form to report such changes. Supporting documentation must also be provided to Carle Benefits within the 31 day time frame. 
• Dependent Eligibility Documentation will be required if adding a spouse or dependent child to the
  plan.

The Carle Dental Plan is administered by Health Alliance Medical Plans and your benefits can be managed through Hally.
