Obtaining My Health and Dental ID Cards

There are many ways to obtain your Member ID for Health and Dental elections

In general, once a team member completes New Hire Enrollment, Open Enrollment or submits a Benefit Change Form for a Qualifying Life Event, along with any applicable documentation, Health and/or Dental Member ID cards are sent out by mail to the address provided in Lawson Self-Service. 

  • It may take up to two - three weeks for the Health/Dental Member ID card to be received in the mail. 
  • For enrollments completed during Open Enrollment, Member ID cards generally arrive in the mail prior to the start of the new year. 
  • If you have an appointment or a prescription refill needed prior to receiving your ID card, call Health Alliance Customer Service at 1-800-322-7451 and they can provide you the information from your ID card for your needs.

Another way to Obtain a copy of your Member ID cards is by logging into your online account on MyChart the Health Alliance web portal. 

You will need your Member ID information to set up your account if you have not logged in before. If you are setting up your online account for the first time but don't have your cards yet, please contact Health Alliance Customer Service at 1-800-322-7451. The log in and set up options are located in the right side of the web page as shown below:

Once logged in you will go to the Menu Icon in the upper left corner of the page:

From there you will be given the option of which plan you would like to view or print a card for. Once you make selection your Member ID card (front and back) will appear on the screen. If you would simply like a new card mailed to you, you may also send a message to Health Alliance using the Messages option in the Menu:

You may also view your Member ID cards by using the MyChart Mobile App available for Apple and Android users! The location of your ID Cards will be the same as it is with these instructions.
