Patients who want an account can ask for an activation code during their clinic visit.
This code lets you log in and create your own username and password. An activation code can also be found on your After Visit Summary or in an email or text message from your Carle provider.
If you have an activation code:
- Visit the MyCarle New User Signup page and search for Carle
If you do not have an activation code:
- Visit the MyCarle Request an Activation Code page.
